After installing the XP patch it makes a new .icd file and .exe. I added them in the zip file below. Hope that will do.

Here you have setupreg.exe. You have to place it in your MM2 folder and run it.

Usually the cracked versions have a setupreg.exe in the MM2 folder. If you run that program, MM2 will be added to the registry so that the graphics patch can find the MM2 installation path.

It's just MM2CT that can't convert those sidewalks properly. MM2CT expects sidewalks to be like this (same number of vertices on inside and outside of sidewalk):
